Tight shoulders are one of the most common mobility complaints I encounter — both in yoga students and in training clients. And the most common approach to addressing them is to stretch the shoulder itself: chest openers, doorway stretches, cross-body arm pulls.

These feel good. But they often don’t solve the problem. That’s because tight shoulders are frequently a thoracic spine problem in disguise.

The Anatomy of Shoulder Mobility

The shoulder is not a single joint — it’s a complex of four joints working together: the glenohumeral joint (ball and socket), the acromioclavicular joint, the sternoclavicular joint, and the scapulothoracic interface. Full overhead shoulder mobility requires all of these to work in coordination.

Here’s what most people miss: the scapula (shoulder blade) must rotate upward and tilt posteriorly to allow the arm to reach fully overhead. And the scapula sits on the thoracic spine. If the thoracic spine is excessively rounded — as it tends to be in people who sit at desks, look at screens, or spend time hunched forward — scapular movement is compromised, and the shoulder joint takes on compensatory stress.

What Thoracic Kyphosis Does to the Shoulder

Thoracic kyphosis is the forward rounding of the mid-back. A modest kyphotic curve is normal and healthy. Excessive kyphosis — whether from prolonged sitting, habitual posture, or insufficient thoracic mobility — restricts the scapula’s ability to move properly on the rib cage.

When scapular mechanics are compromised, the glenohumeral joint has to work harder to produce overhead motion. This results in impingement — where soft tissue structures get compressed between bony surfaces — rotator cuff irritation, and a general limitation in overhead range. You feel it as a tight, pinchy shoulder. The source is often 10 inches south of where you feel the pain.

How to Address It

Thoracic extension over a foam roller is one of the most effective and immediate interventions. Lay a foam roller perpendicular to your spine at mid-back level, support your head, and extend gently over the roller. Work up and down the thoracic spine in small segments. Even three to five minutes produces a measurable improvement in overhead range.

Thread-the-needle stretch — from a quadruped position, threading one arm under the body to rotate the thoracic spine — improves rotational mobility and is particularly valuable for people with desk-bound postures.

Serratus anterior strengthening is underappreciated. The serratus anterior is the muscle that holds the scapula against the rib cage and assists in upward rotation. When it’s weak, the scapula wings outward and overhead motion suffers. Wall slides and serratus push-ups target it directly.

Overhead reaching drills that combine thoracic extension with shoulder flexion train the body to integrate the two, producing more functional carryover than isolated stretching.

Poses like Warrior I, Upward Dog, and any overhead bind require sufficient thoracic extension and scapular mobility to be practiced safely and effectively. Students who struggle to open their arms overhead in Warrior I are often not fighting a shoulder problem — they’re fighting a thoracic restriction that no amount of shoulder stretching will resolve.

Address the spine first. The shoulder usually follows.

When to Seek Help

Shoulder pain with specific mechanism of injury, pain that wakes you at night, or pain accompanied by weakness or neurological symptoms warrants evaluation. Most mobility-related shoulder restriction responds well to the interventions above — but rule out structural issues if symptoms are acute, severe, or unresponsive to conservative care.

Shoulder mobility is a whole-body conversation. If your overhead reach is limited, look up the chain — to the thoracic spine, the scapular stabilizers, and the way your posture distributes load across the system. Fix the foundation, and the shoulder often opens on its own.
